Nurse Manager
Harbor Haven Health & Rehabilitation is Fond du Lac County’s skilled nursing facility and is a leader in offering short-term rehabilitative services, as well as caring for those with long-term care needs. We are a CMS five-star facility, serving the medical, psychological, social, and spiritual needs of each individual resident that we serve.
Examples of Duties:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
- Monitors and directs functioning of nursing staff to ensure compliance with State and Federal regulations, codes, and facility policies.
- Coordinates in-house transfers, new admissions, and discharges. Oversees physician’s plan of care, and the coordination of achieving short-term rehab and long-term care goals at Harbor Haven Health & Rehabilitation.
- Manages and educates staff. Ensures nursing care is delivered in compliance with regulations, and achieves best practice standards. Coordinates and completes auditing of resident care and prevention of negative events/outcomes.
- Assists with audits and reviews of policies and procedures for nursing services as directed by the Director of Nursing.
- Acts as Nurse Supervisor per DHS 132, when indicated.
- Conducts care plan meetings as needed. Focus on individual needs and preferences, plans with IDT to ensure that post-acute care goals are met according to resident preference and that resident is prepared for successful discharge to the extent that it is possible.
- Provides or assists with regular programs of instruction designed to meet the needs of the employees of Harbor Haven Health & Rehabilitation. Familiarizes staff with new procedures, policies, and changes in patient care techniques.
- Monitors unit activities.
- Conducts probationary and annual performance evaluations on registered nurses, licensed practical nurses, and nursing assistants, as directed.
- Takes calls on an as needed basis for the Director of Nursing in his/her absence.
- Takes call to assist with resident care on a rotating basis of every third week or as otherwise directed.
- Acts as interim Director of Nursing in his/her absence, if directed.
- Keeps current on all related training and regulation requirements.
- Conducts and assigns audit and skill reviews with Nursing staff in order to ensure that quality care tasks are being performed, that staff have received all teaching/training needed to competently perform their duties, and in order to meet the State and Federal regulatory requirements.
- Actively participates in QAPI process. Provides a quarterly report on infection control rates and other information as directed/necessary.
- Coordinates care plan needs with attending physicians, and maintains lines of communication with allied professionals as it pertains to resident care.
- Direct oversight of infection prevention of staff, residents, and visitors as it pertains to DHS/CMS regulation.
- Completes the MDSs and care plans for all resident care units, as directed.
- Coordinates an effective immunization program in the facility for staff and residents based on the facility’s policies.
- Completes required DHS and CMS infection control reporting to NHSN or other required reporting sources.
- Maintains illness-tracking lists for all staff and residents, communicating to all departments when an outbreak has occurred and implements appropriate steps. Maintains outbreak tracking, mapping and summary completion.
- Obtains records for employee health files, and maintains related documentation.
- Assigns staff training on the electronic medical record as it pertains to CNA charting.
Desirable Knowledge and Abilities:
Knowledge and sensitive to the needs of post-acute and long-term care residents, knowledge of Federal and State regulations affecting operations of Harbor Haven Health & Rehabilitation, and of supervisory principles. Ability to provide leadership to staff, to maintain good team relationships with staff, to communicate effectively orally and in writing, and to interact effectively with patients and families. Knowledge of Medicare/CMS rules and MDS/RAI processes is highly desirable. Ability to present orally and in writing, information that is to be disseminated to staff. Knowledge of Infection Prevention best practices and regulatory guidance.
Training and Experience:
Bachelor’s degree in Nursing preferred, but not required, as well as registration as a professional nurse in the State of Wisconsin, and 2 years of geriatric and/or short-term rehab nursing experience. Experience coordinating an Infection Prevention Program in a Skilled Nursing Facility preferred.
